Central Racine County Health Department Community Health Improvement Plan

Local health departments are required to develop a community health improvement plan (CHIP), as specified under Wis. Stats. 250.03 (1) (l).

The goal of our CHIP is to take information gathered in the Community Health Assessment 2014, together with input from our community members, the Board of Health, and governmental, business and community agency stakeholders, and translate it into a meaningful roadmap to improve our community’s health together. This plan is meant to be a community vision for health focus areas that our community would like to see improved. The process of putting together the CHIP highlighted the health issues and conditions that currently resonate most strongly with community partners and residents. These issues include: Child Abuse and Neglect; Mental Health; Alcohol & Prescription Drug Misuse; Infant and Child Mortality; and, Obesity. For each of these we examined the impact and data, and we developed goals and intervention strategies to ultimately provide a roadmap for our health department’s work to address critical health issues.
