Participants must be 18 years or older and a resident of Caledonia, Dover, Mt. Pleasant, North Bay, Wind Point, Elmwood Park, Norway, Raymond, Rochester, Sturtevant, Union Grove, Yorkville, City and Town of Burlington, or Village and Town of Waterford. *If you live in the City of Racine, please contact the City of Racine Health Department for programs available to you. |
Week 1 - Initial Visit: Meet with a member of our team to receive a free blood pressure cuff, learn how to take regular readings at home, and receive heart-health resources.
Week 2 - Phone Check-in: Discuss your readings and set a personalized health goal with a member of our team.
Week 3 - Optional Check-in: Receive additional support and discuss your readings with a member of our team.
Week 4 - Final Review: Meet with a public health nurse to review progress, answer questions, and plan next steps.
